Job Description Pride Health is currently seeking candidates for Physical Therapist that will provide support for our client's healthcare systems in Grand Rapids, Michigan. This is a contract assignment with a high likelihood of extension and a great way to start working with a top-tier healthcare organization! Job Description: Job Title: Physical Therapist Location: Grand Rapids, Michigan Schedule : 40/week Shift: 8-hour days Contract Length: 13 Weeks Pay Range for Traveler: $2,600.00- $2,700.00 Weekly DOE Job Summary: Provide physical therapy services to patients of all age groups Develop, implement, and evaluate individualized Plan of Care (POC) Collaborate with physicians and interdisciplinary team Key Responsibilities: Perform clinical assessments to determine patient needs Develop and revise Plan of Care based on ongoing evaluations Document progress notes, phone orders, and clinical records Treat neuromuscular, neurological, and orthopedic conditions Educate patients/families on therapy techniques and equipment use Recommend and adapt durable medical equipment for home setting Supervise team members as applicable Follow infection control and universal precautions Travel locally as required Required Qualifications: Current State Physical Therapist License CPR Certification Valid Driver’s License, reliable transportation, and insurance 1–2 years of experience in acute care or rehabilitation Bachelor’s Degree in Physical Therapy (APTA-approved program)
