Job Description Job Description Nurse Practitioner - PRN Overview: The Nurse Practitioner - PRN is responsible for performing in-home health evaluations for adults and seniors, assessing medical histories, ensuring medication compliance, and working alongside healthcare teams to enhance patient outcomes. This position provides flexible scheduling options and competitive compensation based on the number of visits completed, promoting a healthy work-life balance. Care is delivered in patients' residences or skilled nursing facilities, aiming to bridge care gaps through thorough assessments and follow-ups. Responsibilities About the Role During visits that may last up to one hour, our providers assess a member’s current health status, medical history, medication adherence, social circumstances, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Comprehensive Health Assessment (CHA) enhances care quality and allows us to address multiple care gaps in a single visit. Our providers: Perform assessments for adults and seniors, including medical history, diagnosis, treatment, health education, referrals to physicians and case managers, follow-ups, and thorough documentation in line with our protocols. Administer and order necessary point-of-care and lab tests. Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed. Work with Primary Care Physicians (PCPs) to educate patients and ensure appropriate follow-up. Deliver services at the member’s residence, whether in their home or in Skilled Nursing Facilities. Benefits of Joining Our Team Flexibility – Schedule visits according to YOUR availability. Assessments are conducted from Sunday to Saturday, between 8 am and 7 pm. You can choose to work during the day, evening, or weekends based on your preferences. Competitive Compensation – Your earnings are based on the number of assessments completed. Increase your income potential by being available for more visits and traveling to see members outside your local area. Those averaging 20 to 100 visits per month can earn between $1,800 and $11,500 , depending on the per assessment rate of $90 to $115 based on your state. Additional Benefits – Includes mileage reimbursement, a 401(k) plan, employee referral bonuses, and limited voluntary benefits. Education and Support – We prioritize your development! Along with comprehensive onboarding training, you will be assigned a mentor, receive personalized feedback, and have free access to continuing education opportunities. Licensure Support – If business needs arise, we will assist you in obtaining additional state licensure and credentialing in neighboring states or others you may wish to visit while conducting health assessments. Full-Time Opportunities – Our PRN providers have the option to transition to full-time roles within their home location or in other states or regions, along with a full-time sign-on bonus. Qualifications Essential Skills & Experience A Master’s Degree or equivalent experience, along with successful completion of NP licensure. Current NP licensure in the state of practice, including prescription authority or the ability to obtain it as required. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. Preferred one year of NP experience; new graduates are encouraged to apply! A valid state driver’s license and proof of adequate automobile insurance for your state of residence. Strong computer skills and familiarity with health/medical record software. Excellent verbal and written communication abilities.
