We’re looking for a Hospice RN – Regional Support to join our team. In this dynamic role, you’ll provide direct care to patients and support to families, while also serving as a floating clinician—temporarily assigned to different hospice branches in W MI (could include areas of Grand Rapids, Fremont, Traverse City, Okemos, Ann Arbor, Jackson and Portage, MI) based on census needs or staffing gaps. As a Hospice RN – Regional Support, You Will: • Deliver high-quality, compassionate nursing care for hospice patients • Manage and implement individualized plans of care • Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) • Assess and address patients’ physical, emotional, spiritual, and environmental needs • Accurately document all care provided and ensure compliance with hospice protocols • Educate families and caregivers on care instructions, medications, and safety • Participate in the on-call rotation and provide after-hours care as needed • Collaborate with physicians, referral sources, and the care team • Assist with referrals, regulatory documentation, and payer compliance • Travel to various Gentiva hospice locations as assigned to fill urgent needs Schedule & Travel: This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include: Option 1: Overnight travel required to cities within a 100-mile radius. Option 2: Frequent overnight travel to multiple states is required. Option 3: Regional travel with overnight stays based on assignment needs. A company fleet car is provided for eligible employees.
